feat(auth): 회원가입 시 휴대폰·전화번호 선택 입력 추가
마이페이지·관리자 회원 폼에는 이미 연락처 필드가 있으나 회원가입 폼에만 빠져 두 화면이 불일치했다. DB·모델·Resource는 기존 존재하므로 회원가입 경로에만 검증·저장·입력 UI를 연결한다. 연락처는 선택 항목이다.
